Birth Certificates and Immunizations

Birth Certificates and Immunizations are required for your child to attend school in the state of Indiana.

The Indiana Department of Education memorandum on enrollment restrictions and denial of enrollment states:

 Lack of birth certificate – A student must have a birth certificate or other reliable proof of students date of birth. Failure to provide a birth certificate or other reliable proof of the date of birth within 30 days of enrollment may result in a report to the Indiana Clearinghouse of Missing and Exploited Children. 

If you have not turned your child’s birth certificate or shot record, please send these documents to the office as soon as possible or contact the office at 812-288-4865.
